Over-The-Counter Medication Authorization Form - Please Review

The New York State Education Department requires written approval from both your child’s healthcare provider and a parent or guardian for any medication given in school. This includes prescription medications and common over-the-counter (OTC) items such as Vaseline, ChapStick, and similar products.

If your child needs medication during the school day:

  • An OTC Medication Authorization Form must be completed by both you and your child’s healthcare provider.
  • You must supply all prescribed and OTC medications specific to your child.

Our school nurse also keeps a limited supply of commonly used OTC products. These may be used only if you give additional permission.

The OTC Medication Authorization Form must be signed and initialed by both you and your child’s healthcare provider. Once completed, it allows the school nurse to administer or apply approved OTC medications during school hours.
